Professional Skilled Nursing Care and Rehabilitation Services
Clinical Care
We are one of few facilities that has a hands-on Medical Director, a physician who has been coming to see our residents himself every single week since 2010. Our Medical Director communicates directly to our Director of Nursing, who oversees our team of Registered Nurses, Licensed Practical Nurses, Certified Nursing Assistants, and resident assistants that provide 24/7 direct care to all our residents.
Our compassionate team is dedicated to providing exceptional skilled nursing care around the clock. Our commitment to high-quality skilled nursing care is evident in our dedicated medical team. For our residents' comfort and convenience, we also provide in-house access to supplementary medical services such as Psychiatrists, Therapeutic Behavioral Services, Podiatrists, Audiologists, Dentists, and Ophthalmologists.
Assisted Living
The need for memory care is significant for our aging population. Singleton Health Care Center will address that need by providing our Assisted Living residents with intensive specialized care for people with memory issues.
Our goal is to keep each resident in the least restrictive, most enriching, environment possible, allowing for the maximum level of independence. In addition to our skilled nursing/long term care unit, Singleton Health Care is a licensed residential care facility or commonly referred to as Assisted Living. Our assisted living certification comes through the widely recognized Western Reserve Area Agency on Aging (WRAAA aka “Waiver program”). We accept residents with Medicaid as their primary insurance into our assisted living unit, provided that they meet the appropriate level of care.
As an assisted living waiver facility, we provide a private room with a private bath and shower. We provide privacy and independence to our Assisted Living residents, but with the comfort of knowing there is nursing staff on premises available 24 hours a day. Residents receive three home cooked meals a day as well as two snacks, and a full scale laundry service. A full daily schedule of activities and outings is available if desired. The facilities Medical Director has a close relationship with our residents and can be reached at any time with any inquiries or concerns. We have had excellent results from our annual state compliance surveys and we are proud to be citation free from the WRAAA annual survey.
We are proud of our diverse population and all assisted living staff members are continuously trained in mental health awareness and treatments. Being a small facility allows us to work closely with the residents and their families and maintain personal relationships and ensure all needs are being met. We offer the same high standards of compassion and dedication as our skilled nursing care program. This personalized approach is what distinguishes our skilled nursing care and assisted living programs.
Dietary
Our dietary service provides nutritious, home-cooked meals prepared from scratch in our kitchen under the supervision of a full-time dietary supervisor and licensed dietitian. Our cooks have been working at Singleton for 20 plus years and many of them cook and bake with such mastery that they don’t even use recipes. At least one-to-two times per month we have a special feast for holidays and birthday meals where residents get to choose their favorite foods for the whole house to enjoy.
Our kitchen provides alternative meals for any special diets needed, whether due to religious, preferential, or medical necessity. Our Dietary Manager of 35+ years was awarded OHCA’s Excellence Award in 2022 and was featured in the documentary “People Worth Caring About”. Nutritional support is a core pillar of our skilled nursing care.
Activities
Our activities department is dedicated to enriching all the areas of our residents’ lives. We provide daily programs, created based on person-centered interests that engage our residents mind, body and soul. We offer opportunities to participate in beloved hobbies and also encourage trying new things. We have a large focus on community partnerships to ensure that our residents are still a vital part of their communities. We collaborate with churches, schools, sororities and other organizations to create a diverse and vibrant schedule of activities.
We like to take advantage of our great location and visit cultural landmarks and other wonderful local spots in the City of Cleveland. We ensure that there is something for everyone to do and be involved in to contribute to their overall feeling of purpose in the Singleton Family. We believe a holistic approach, including engaging activities, is essential to skilled nursing care.
Therapy
At Singleton Health Care, we offer comprehensive rehabilitation therapeutic services which include Physical, Occupational, and Speech Therapy, all crucial components of excellent skilled nursing care. In addition we offer restorative care to promote and maintain the maximum level achieved during their skilled therapy stay. We also provide specialized skilled nursing care for post-acute recovery. Additionally, respite care is available. Our therapists have been working onsite at Singleton for almost 20 years, providing consistent skilled nursing care.
Transportation Services
Residents may use in-house provided medical services or maintain their relationship with community doctors if they choose. We provide free transport in our wheelchair accessible Promaster Van, along with an escort, to and from medical appointments as well as facility activity events in the community. This support is part of our comprehensive skilled nursing care model.
Housekeeping
At Singleton Health Care Center we take cleaning seriously! Our housekeeping department provides daily cleaning services to keep our facility sparkling and odor free. We believe in using high quality cleaning products to provide not only a pleasant environment but also one that ensures effective infection control, which is vital in providing skilled nursing care. Our Maintenance Director of 20+ years was awarded OHCA’s Excellence Award in 2025.
Laundry
We look at personal resident laundry and facility laundry, such as linens and towels, as two distinctly different entities. In order to provide a plentiful amount of clean and hygienic linens, we utilize an outside laundry company that delivers fresh linen every day. For the residents personal clothing however, we use a unique in-house service that ensures quality and care to the same level as one receives at home in the community.
